What physical changes after 45 affect the way we walk?

Cartilage gradually wears down, ligaments lose some elasticity, and muscle mass begins to decline. Deficiencies in Vitamin D and iron make all of it more noticeable. The knees and hips feel it first because they carry most of the load — but in my experience the story begins much lower down. The foot is the body's first point of contact with the ground, and the shoe is what comes in between.

How do our feet change as we age?

Most people think the foot has a single arch. It has three — medial, lateral and transverse — and together they work as a spring, what we call the Windlass Mechanism. It absorbs impact, stores energy, and propels you forward with every step. As we age, the tissue supporting it loses elasticity and the arches begin to flatten. The foot gets wider, absorbs less shock, and the discomfort often travels up to the knees, hips and lower back.

Why does balance become more challenging with age?

Added abdominal weight and changes in posture both push the body's centre of gravity forward. When your centre of gravity moves forward, your body is constantly working to bring itself back into balance. Walking starts to feel less stable, and that is one reason the risk of falls rises with age.

What do most shoes get wrong for this age group?

The biggest issue is that most shoes aren't designed for this stage of life. They're built around athletic performance or generic comfort. Wider feet, changing posture, reduced stability and declining arch function need a different design approach than what most footwear currently offers.

If you were designing a shoe for someone over 45, what would matter most?

Three things, in this order.

01 — A wider, anatomical toe box. Feet become wider with age and conditions like bunions become common. The toes need room to spread naturally rather than being squeezed into a narrow shape.

02 — Support for all three arches. Most supportive footwear only addresses the medial arch. Effective support should work with the entire structure of the foot to assist its natural spring.

03 — A firm heel counter. The heel keeps the foot aligned during walking. A firm counter provides stability and holds the foot in position through every step.

When patients come to you with foot or lower-limb issues, what do you typically see?

Most of it comes down to arch collapse and alignment. When the arch begins to collapse — what we call a pronating foot — the impact doesn't stay in the foot. It sets off a chain reaction through the ankle, the knee, and even the lower back. That's why I always start by looking at how the foot is functioning, because that's where most problems begin.

What does most footwear get wrong for people over 45?

People either wear shoes that are fashionable and offer very little support, or highly cushioned shoes that feel comfortable but don't address the structural needs of the foot. Comfort and support are not always the same thing. Some brands do build in genuine arch support, but they sit at the premium end of the market. That leaves a gap.

03 · Heel-to-toe drop

We've reduced the heel-to-toe drop to around 5–6mm. What's your view on that?

That's the right direction. A high heel drop, where the heel sits significantly above the forefoot, is essentially like wearing a small heel all day. Over time it shortens the calf muscles, alters your gait, and shifts load onto the forefoot in an unnatural way. A lower, more controlled drop encourages a more natural foot strike and better posture — and that matters more after 45, when the gait is already under stress from muscle weakening and joint change.

04 · Wide toe box

We've made the toe box wider and more anatomical. Does that matter as much as we think?

Absolutely. A narrow toe box compresses the toes and contributes to bunions, hammer toes and forefoot pain. The forefoot naturally widens with age, so a cramped shoe becomes more of a problem over time, not less. Giving the toes enough room to sit and spread naturally reduces unnecessary pressure at the front of the foot. And a wider toe box doesn't have to look clinical — you can get the shape right and still design a shoe that feels modern and everyday.

05 · Hands-free entry

We've added hands-free entry so people don't have to bend down. Is that a real problem or a convenience?

That's a detail many people underestimate. Bending down repeatedly, especially while balancing on one foot, becomes genuinely difficult as people get older. If putting on supportive footwear feels like a chore, people are less likely to wear it consistently. A well-designed slip-on removes that barrier, and combined with a secure fit it makes supportive footwear easy to use every day. It's a small design detail with a meaningful effect on daily life.

What are the most common foot complaints you see among Indian patients, and do they differ by age or gender?

Heel pain, plantar fasciitis, and knee pain linked to poor foot alignment are what I see most. Age plays a clear role. Patients past their mid-forties often come in with arch collapse or early arthritis, simply from years of wearing shoes that gave their feet no real support. Gender matters too. Women often struggle with bunions and toe deformities after years of narrow or heeled footwear, while men more commonly present with plantar fasciitis from flat, cushionless slippers.

What is the biggest mistake people make when buying their daily shoes?

Most people buy shoes for how they look, not for how they support the foot. I ask my patients what made them pick the shoes they are wearing, and the answer is almost always price or style. Very few think about arch support or cushioning until they are already in pain. That single habit, choosing look over support, sits behind a huge number of the cases I treat.

How do traditional Indian footwear choices affect long-term foot health?

Chappals and flat sandals feel comfortable in the moment, but they give the foot almost no support. Over years of daily wear that leads to overpronation, plantar fasciitis, and added strain on the knees. Heavy festive heels do damage in a different way. They throw off your posture and balance for a few hours at a time, but repeated over decades that adds up to real, lasting damage to the joints.

What are the biggest problems with the footwear available in the market today?

Most shoes in the market are designed to look good on a shelf, not to support a foot through eight hours of walking. I see narrow toe boxes that crowd the toes, cushioning that breaks down within weeks, and almost no attention to arch support. Very few brands design for how feet actually change as we age. Feet widen, arches flatten a little, and most shoes simply do not account for that.

05 · Three essentials

If you could mandate three structural features in every daily-wear shoe, what would they be?

If I could mandate just three things, it would be proper arch support so the foot does not collapse inward, a toe box wide enough for the toes to sit naturally, and a firm, cushioned heel that absorbs shock without letting the foot wobble. Get those three right and you prevent a huge share of the problems I see in clinic.

06 · Cushioning vs support

Is a super-soft memory foam sole actually good for the feet?

This is something people get wrong often. A super soft memory foam sole feels lovely in the shop, but it does not give the foot the stability it needs. As we get older, an overly soft sole can actually increase the risk of losing balance. What the foot needs is a balance of the two: enough cushioning to absorb shock, sitting on top of firm, structured support that keeps the foot properly aligned. Comfort and support have to work together, not one at the cost of the other.

07 · Breathability

How does Indian heat and humidity affect foot health?

Our climate is tough on feet. Heat, humidity and the monsoon create ideal conditions for fungal infections, sweat-related skin problems and odour, particularly in shoes made from non-breathable material. I tell my patients, especially those managing diabetes, that breathable, moisture-wicking material is not a luxury. It is a genuine health need. Moisture trapped against the skin for hours every day is how small problems turn into bigger ones.
